Laser toenail treatment

Laser treatment of fungal toenails — one of our named specialty services. Ask us whether it is right for you at your visit.

Keryflex prosthetic nail application

Keryflex is a nail resin used to lengthen or re-shape the nail. It lasts up to 6–8 weeks before a fill-in is needed, restoring damaged toenails with a medical-grade gel enhancement built on top of the existing nail. It can also be used during antifungal treatment to encourage healthy nail growth.

Custom orthotics

Custom orthotics designed to support your feet and address the biomechanics behind heel pain, flat feet and other everyday foot problems.

Medical pedicures

A medical pedicure is a non-invasive procedure performed by a highly skilled pedicurist. These services are guaranteed to be as safe and hygienic as it gets, and are customized to fit your specific foot-care needs. Foot conditions and issues are addressed and you are provided with the highest level of cosmetic care. Dr. Carly Chapman

Doctor of Podiatric Medicine

Dr. Chapman received her Bachelor of Science from Presbyterian College, where she majored in Biology and minored in Chemistry and English, graduating Summa Cum Laude. She earned her doctorate from Temple University School of Podiatric Medicine and completed a three-year residency at the Atlanta Veteran’s Administration Medical Center.

She is a member of the American Podiatric Medical Association and the Forensic Podiatry Society, and is a certified Wound Specialist Physician with the American Board of Wound Management. She previously worked at the Columbia Foot Clinic in high school, and is the granddaughter of Dr. Suber.

Dr. Todd Davis

Doctor of Podiatric Medicine

A native of Florence, SC, Dr. Davis graduated from the California College of Podiatric Medicine in San Francisco in 1997. He completed his residency at VAMC in Richmond, VA, and a fellowship at the Ambulatory Surgery Centers in Tempe, AZ, and has been on active surgical staff for 17+ years at Lexington Medical Center.

Dr. Davis first joined the Columbia Foot Clinic in 2001 and has been back with the practice since 2016. An avid runner who has completed many full marathons, he treats all types of foot problems, medically and/or surgically.

Dr. Daniel Miller

Doctor of Podiatric Medicine

Dr. Miller attended Loyola University New Orleans and earned his Doctor of Podiatric Medicine from the Dr. William M. Scholl College of Podiatric Medicine in 2012, publishing work in the biomechanics journal Gait and Posture. He completed a three-year surgical residency at Loyola University Medical Center / Edward Hines VA Hospital in Chicago.

His special interests include diabetic limb salvage, wound care, reconstructive foot and ankle surgery, trauma, pediatrics and chronic heel-pain treatment. He is a member of the American Podiatric Medical Association and the American College of Foot and Ankle Surgeons.

We are so glad you are considering us for your foot-care needs. At Columbia Foot Clinic we strive to give our patients a world-class experience. Please come to your visit with your forms completed so your appointment proceeds in a timely manner.

  • 2 Your co-pay for a specialist
  • 3 A photo ID (driver’s license, military ID, etc.)
  • 4 A list of any medicines you take regularly, including OTC
  • 5 A list of any allergies you have
  • 6 Your completed New Patient Form (download below, or complete it at our office)

Please arrive 15 minutes early to complete forms if you have not done them ahead of time at home.

Appointment policies

  • There is a $50 fee for cancellations with less than 24 hours’ notice.
  • We have a 10-minute grace period on all appointments. If you are later than 10 minutes, we may need to reschedule your appointment.
